Registering with the practice
You can now register online or you can download a registration form.
If you are able please scan the paper form and return to syicb-rotherham.wickersleymedicalpractice@nhs.net or place the form in an envelope and post through our external letterbox which is accessible at any time.
Proof of Photo ID and something that confirms your address would be beneficial not essential. This can also be emailed or posted to us with your form (please do not send originals)

Your medical notes will be sent to the practice from your previous GP. You also have the right to express a preference to which GP or nurse you preference to see either generally or in relation to a particular condition. We will record your preference for future appointment bookings.
